Instructions: From the FIND A PROVIDER Search Screen, you need to select:

1. The desired Specialty (default set to General Dentist)

2. The Number of Results (default set to 30)

3. Search by either Address or Dentist Name default set to Address).

To begin the search, press the 'Get Provider Information' button.

When using Address to search, both ZIP code and mile radius are required.

• Searching by ZIP code without an address will return randomly selected dentist locations within the mile radius chosen. Additional dentist locations may be found by conducting another search using the same ZIP code. Expanding the mile radius may also return additional dentist locations.

• When both a ZIP code and street/city address are used, the results list begins with the closest location to the address entered.

When using Dentist Name to search, State and either Dentist Last Name or Business Name are required.

• Using both the Dentist Last Name and Business Name will narrow the results

• Including the Dentist’s First Name will further narrow the results

Renaissance makes every effort to provide accurate and up-to-date information about participating network dentists. It is however recommended to confirm with the dental office when scheduling an appointment that the dentist is currently participating.
